Employee training failures associated with organizational scandals

I'm writing a paper on auditing the training function in organizations. I'm trying to identify examples of when either the lack of training or the wrong training was associated with some kind of scandalous event. Certainly, environmental catastrophes would be a prime example.

If anyone can offer some ideas of situations and/or references, I would greatly appreciate it. Thanks.
